L-0377 - DECLARATION VERIFYING IDENTITY OF DEFENDANT RE: AUTOMATED ENFORCEMENT SYSTEM CITATIONS. This form is used by law enforcement personnel in Orange County, California, to formally verify the identity of a defendant who has been issued a citation through an automated enforcement system, such as a red-light camera. It is completed when the defendant has failed to appear for arraignment. An authorized employee of the law enforcement agency uses this form to certify that they have personally compared the photograph captured by the automated enforcement system with the Department of Motor Vehicles (DMV) photograph of the named defendant. The declarant affirms under penalty of perjury that, to the best of their knowledge, the person in the citation photo and the DMV photo are the same individual.
