CV\E–204 - NOTICE OF CHANGE OF HANDLING ATTORNEY WITHIN-FIRM. This is a Sacramento County Superior Court form used when a law firm or government agency needs to update the court about a new handling attorney on an existing case. The form allows the attorney to identify the firm, note which attorney previously appeared, and request that the outgoing attorney be removed from the court’s service list. It also directs the court to send all future notices, pleadings, and case communications to the newly designated attorney. A required proof of service section confirms that all parties or their counsel were properly notified of the change, and the declaration is signed under penalty of perjury.
