CM111 - PROMISSORY NOTE. This form is used in the Court of Chancery of the State of Delaware when a guardian of the property of a minor or a person with a disability has borrowed funds from the guardianship account and is required to formally acknowledge the debt and agree to a repayment plan. By signing this document, the guardian promises to repay the specified amount each month, beginning on a designated date, until the full amount borrowed is repaid. The guardian must submit proof of each monthly deposit to the Register in Chancery by the 15th of the month. If proof is not provided, the guardian will be required to appear in court. The form also states that no future petitions to expend funds from the guardianship account will be considered by the court until the borrowed amount is fully repaid.
