This form is used in the District Court of Maryland by individuals who wish to view official court records, such as case files related to traffic, criminal, civil, or other legal proceedings. The applicant must provide identifying details including the case number, trial date, and case name, along with the names of the defendant and, in civil matters, the plaintiff. If the request involves multiple cases, an addendum form (DC 34A) should be attached. The form outlines fees for obtaining copies: $0.50 per page for photocopies and $5.00 per document for certified copies. For unserved arrest warrants or charging documents, access is restricted to certain authorized parties, such as state’s attorneys, peace officers, correctional officers, attorneys for the defendant, or judicial officers, and proof of that status may be required.
