JD-CL-137 - REQUEST FOR HEARING, DENIED APPLICATION FOR ISSUANCE OF SUBPOENA. This is a Connecticut Judicial Branch form used by self-represented parties whose Application for Issuance of Subpoena (JD-CL-136) has been denied. The form allows the applicant to request a court hearing so a judge or family support magistrate can review the denial and determine whether a subpoena should be issued. It may be used in civil, housing, family, family support magistrate, or criminal/motor vehicle matters pending in Connecticut Superior Court. Following submission, the clerk schedules a hearing and notifies the applicant of the date, time, and location.
