JD-JM-203 - MOTION PERMANENT LEGAL GUARDIANSHIP. This is a Connecticut Superior Court (Juvenile Matters) form used to request or modify a permanent legal guardianship arrangement for a minor child. The form allows a party, such as the Department of Children and Families (DCF), a guardian, or another authorized individual, to ask the court to appoint a permanent legal guardian, remove an existing guardian, or appoint a successor guardian.
