MAD-CIV-023 - DEMOGRAPHICAL INFORMATION. This is a local form used by the Madera County Superior Court to provide identifying details about an individual when the court is issuing a warrant or attachment for a defaulter. The form collects personal and physical identifying information—such as the person’s name, date of birth, Social Security number, driver’s license information, physical description (height, weight, hair color, eye color), and last known address—to assist law enforcement or relevant agencies in identifying the individual.
