FORM 25.0 - APPLICATION FOR ORDER TO DISINTER REMAINS [R.C. 517.24, 517.25, 2108.70 et seq.]. This form is used in the Ohio Probate Court when a person seeks a court order to disinter (exhume) a deceased person’s remains from their current burial site. The form ensures that disinterment is properly reviewed and approved under state law. The applicant must affirm they are an interested person of sound mind and at least 18 years old, specify the current and intended reinterment locations, and provide a list (Form 1.0) of heirs, next of kin, legatees, and devisees entitled to notice. Notices must be sent via certified mail to the decedent’s surviving spouse, heirs, will beneficiaries, and the cemetery, unless waived in writing. The applicant must also attest that the disinterment does not violate the decedent’s religious beliefs, state the cause of death, and confirm the decedent did not die from a contagious disease—or attach a health board permit if applicable.
