Use it to request a good-cause exemption from mandatory e-filing under Supreme Court Rule 9(d)(2) when you cannot e-file—e.g., no internet or computer access at home, no email or payment card, limited English, or unavailable equipment/help. Some filers are automatically exempt and do not need this form: those in jail/prison, filing into juvenile cases, or unable to e-file due to a disability.
