Mental Health Diversion Progress Report (MHDRPR}

MHDPR - MENTAL HEALTH DIVERSION PROGRESS REPORT [PENAL CODE § 1001.36]. This form is used to document and report the progress of a client participating in a Mental Health Diversion program. It is completed by the client’s treatment provider or treatment team and submitted to the court and relevant parties via email. The form captures information including the client’s personal details, case number, reporting period, current diagnosis, medications, and the dates of recent and upcoming appointments. It also assesses the client’s compliance with and engagement in their treatment plan, indicating whether they are satisfactorily meeting the requirements, partially meeting them, non-compliant, or in need of a higher level of care. The form allows for notes about any modifications to the treatment plan or changes in diagnosis and includes a section for additional comments from the provider.
