MHDPR - MENTAL HEALTH DIVERSION PROGRESS REPORT [PENAL CODE § 1001.36]. This form is used to document and report the progress of a client participating in a Mental Health Diversion program. It is completed by the client’s treatment provider or treatment team and submitted to the court and relevant parties via email.
