Description
MEX-171 - EXHIBIT COVER SHEET. This form is used in the Superior Court of California, County of Mendocino, to accompany documents, photographs, or electronic media that are being submitted as exhibits for an upcoming court hearing. The form identifies the submitting party—such as County Counsel, District Attorney, attorneys for involved parties, ICWA representative, CASA, or another authorized individual—and includes the date, time, and department of the scheduled hearing. It provides a space to list and describe each exhibit and confirms compliance with local rule 5.8(c)(5), which requires that all exhibits be served to all parties at least five days before the hearing. If the exhibit includes electronic media, the form includes a section to note the type of media submitted and the date it was delivered to the clerk’s office. Mandatory. www.FormsWorkflow.com
