This form is issued by the Missouri Secretary of State, Commissions Division, for notaries who have changed their residence or mailing address within the same county where they are commissioned. This form updates the notary’s official record without altering the county of commission. Applicants must provide their full name, date of birth, previous and new addresses, county of residence, and current notary commission number. A $5 filing fee is required, payable by check, money order, or credit card (an additional $1.25 applies for card payments).
