Form NFP-104.15R - ASSUMED NAME RENEWAL. This form, issued by the Illinois Secretary of State under the General Not For Profit Corporation Act, is used for renewing the registration of an assumed corporate name. Nonprofit corporations incorporated in Illinois or authorized to transact business in the state must file this renewal to continue legally using their assumed name. The right to use the assumed name is extended until the first day of the corporation’s anniversary month in the next year evenly divisible by five. The form requires the corporate name, state or country of incorporation, and the assumed name(s) to be renewed. A $150 fee per name applies if filed on time, and $155 if late.
