Form JC 12b - PROOF OF SERVICE/NONSERVICE. This form used by the Judicial Circuit – Family Division in the State of Michigan is designed to document the delivery or attempted delivery of court documents in family law cases. This form is not to be used for proof of service of a summons or for publication. It includes sections to record whether service was completed by mail, in person, or was unsuccessful. When service is done by mail, the form requires the server to specify the type of mail used—ordinary, certified, or registered—and to list the names and addresses of the recipients. For personal service, the form must detail who was served, where, and when. If the person could not be located after reasonable efforts, the nonservice section must be completed, including the names of those not served and the reasons why service could not be made.
