Form JC 12a - PROOF OF SERVICE/NONSERVICE. This form used by the Judicial Circuit – Family Division in the State of Michigan documents how legal papers were delivered—or not delivered—in a family court matter. This form is not to be used for proof of service of a summons or for publication. It allows the person serving documents to certify whether service was completed by mail, in person, or not at all. If served by mail, the server must indicate the type of mail used (ordinary, certified, or registered) and provide the names and addresses of the recipients. If served personally, the form must include the names of those served, the location, and the date and time of service. In cases where service could not be completed, the server must explain the efforts made and why the individuals could not be located.
