Form 1 - APPLICATION FOR A NEVADA GAMING LICENSE BY AN INDIVIDUAL. This form is used by the Nevada Gaming Control Board to collect detailed personal and business information from individuals seeking a license to participate in the gaming industry within the state. This application is required for individuals who want to obtain approval as officers, directors, shareholders, key employees, members, or managers involved in either restricted (1–15 gaming devices) or nonrestricted (15+ devices and/or live gaming) gaming operations. The form requires information about the applicant’s identity, contact details, and the entity or gaming establishment where the individual seeks approval. It also collects details about ownership interests, title, the percentage of ownership, and whether the shares were purchased from a treasury or individual. The application must be signed by both the applicant and a current licensee, where applicable, and must be accompanied by a nonrefundable application and investigation fee. This application may not be withdrawn without the permission of the licensing agency. During the course of the investigation of the application, an applicant may be required to undergo a drug test.
