REQUEST TO RELEASE PROTECTED DECEDENT’S REMOVED INFORMATION [§119.071(4)(d)9]. This form is used to request the unredaction of certain personal information from public records that had previously been removed for privacy protection under Florida law. This form is specifically applicable when the individual whose information was protected is now deceased. The affiant, or person making the request, must affirm under oath that the decedent has passed away, as verified by a certified copy of the death certificate, which must be attached to the affidavit. The affiant must also confirm that no statute or court order exists that would prohibit the release of the previously redacted information and that the reason for the request is the death of the protected individual. The form requires the affiant to provide details identifying the specific public records involved, such as the document title, book and page number, and instrument number where the redacted information is located.
