ADOPTION QUESTIONNAIRE ­ STEPPARENTS. This form is used by the Oakland County Circuit Court, Family Division, Adoption Services, as part of the stepparent adoption process. It collects detailed personal, familial, and background information about the child(ren) being adopted, the proposed adoptive stepparent, and the custodial and non-custodial birth parents. The form includes names, addresses, contact details, employment and income status, as well as the living arrangements and relationship history between the child(ren) and stepparent. It also asks for references who can provide character letters to the court, excluding relatives.
