Form 7-15 - PETITION FOR VIOLATION OF ORDER OF DISPOSITION. This is a New York Family Court form used when a youth previously adjudicated as a Person in Need of Supervision (PINS) is alleged to have willfully violated the terms of their Order of Disposition. The petition may be filed by a probation officer, placement agency, or social services representative. It identifies the respondent’s prior adjudication, outlines the conditions of probation or placement, and specifies the acts constituting the alleged violation. If placement or continued placement is sought, the petition must address whether remaining at home would be contrary to the respondent’s best interests, whether reasonable efforts were made to prevent removal, and if applicable, justify placement in a Qualified Residential Treatment Program (QRTP) under SSL § 409-h. It also requires disclosures under the Indian Child Welfare Act (ICWA) and includes separate verification sections for individual and agency petitioners. The form concludes with a request to revoke or modify the Order of Disposition and impose an appropriate remedy under Article 7 of the Family Court Act.
