PLRB-13 - PETITION UNDER THE PENNSYLVANIA LABOR RELATIONS ACT OR ACT 111 OF 1968. This form is used to initiate various types of proceedings before the Pennsylvania Labor Relations Board concerning the representation of employees for collective bargaining purposes. It may be submitted for several reasons: (1) to request a representation election to certify a union as the exclusive bargaining representative for employees in a defined unit, (2) to file a joint request where both the employer and employee organization agree on recognition and the appropriateness of the unit, (3) to request decertification when employees or the employer believe the current representative no longer has majority support, (4) to clarify an existing bargaining unit’s scope, or (5) to amend a previously issued certification. Depending on the type of petition, the form requires supporting documentation such as a signed showing of interest, affidavits, or factual statements. The form collects details about the employer and employee organization, the scope and size of the bargaining unit, any existing representation claims or labor agreements, and other relevant facts. It must be signed and verified by the petitioner and may be jointly signed in the case of joint recognition.
