FORM 15.3 - GUARDIAN'S BOND [R.C. 2109.04(A)(1)] . This form is used by the Probate Court in Ohio when an individual is appointed as the guardian of a ward’s estate. This form serves as a financial guarantee to ensure that the guardian—referred to as the principal—will faithfully carry out their fiduciary duties and manage the ward’s estate in accordance with the law and any additional requirements imposed by the court. The bond amount is set by the court and represents the maximum liability of the guardian and any sureties (guarantors) who agree to be financially responsible if the guardian fails in their duties. The obligation stated in the bond becomes void if the guardian fully and properly performs all required duties. However, the bond remains in effect and enforceable if the guardian acts improperly—such as being negligent, delaying duties, misusing funds, or converting estate assets for personal or unauthorized use. If personal sureties are involved, they must certify that they own real estate in the county, and they must disclose the net value of that property to demonstrate financial capacity to cover potential losses.
