AOC-SP-600 - APPLICATION/NOTICE OF RESUMPTION OF FORMER NAME. This form is used in the Superior Court Division, Before the Clerk, in the State of North Carolina to legally resume a former name following a divorce or the death of a spouse. The form allows an individual to notify the court of their intent to revert to a previous name, such as a maiden name, a surname from a prior marriage, or a premarriage surname. The applicant must provide personal identifying information including their current name, name at birth, birth details, and the name they wish to resume. In the case of a divorce, the applicant must confirm they are a resident of the county or that the divorce was granted there, and indicate the applicable name they intend to resume.
