AOC-CVR-2 - REVOCATION ORDER WHEN PERSON PRESENT. This form is used by a judicial official to revoke a person’s driver’s license or driving privilege when the individual is present at the initial court appearance. This order is issued based on probable cause that the individual committed an implied-consent offense—typically related to impaired driving—and either refused a chemical analysis or registered a qualifying alcohol concentration. The form outlines that the person’s license or driving privilege is revoked for at least thirty (30) days, starting either from the date of the order, the date of license surrender, or, if multiple offenses are pending, until final judgment is entered for all such offenses. The form also includes supplemental findings regarding the actual surrender of the license or confirmation of unlicensed status.
