AOC-CVR-13 - REVOCATION ORDER FOR PROVISIONAL LICENSE WHEN PERSON PRESENT AT INITIAL APPEARANCE. This form is used by the North Carolina District Court to officially revoke the driving privileges of a provisional license holder under the age of 18 who has been charged with committing a criminal moving violation. This order is issued when the person is physically present at their initial court appearance. After finding probable cause that the individual committed such an offense, the judicial official orders the revocation of the person’s limited learner’s permit, limited provisional license, or full provisional license. The revocation begins on the date of the order and must remain in effect for at least 30 days, unless the license is revoked for another reason by the court or the DMV. The person is prohibited from driving during the revocation period and is not eligible for a limited driving privilege. The judicial official must inform the individual of their right to request a hearing to challenge the order and provide the form required to do so. The form also includes clear notice that driving during the revocation period is unlawful, and that the license will only become valid again once the revocation period ends and no other revocations are in place.
