SJPR-006A - ATTACHMENT TO DECLARATION RE: NOTICE OF EX PARTE APPLICATION. This form is used to document compliance with California Rule of Court 3.1204, which requires notice to opposing parties before seeking ex parte relief from the court. This form must be completed by the party submitting the ex parte application to show when, how, and to whom notice of the application was given. The notifying party must indicate whether the notice was provided by telephone, in person, fax, email, or another method, and specify the date and time of that communication. The form also requires a description of the opposing party’s response—whether they opposed, did not oppose, or responded in some other way. If notice was not given by 10:00 a.m. the court day before the requested hearing date, the party must explain the exceptional circumstances that justify the late notice.
