LWC-WC-1000 - ANNUAL REPORT OF WORKERS' COMPENSATION COSTS. This form is required by the Louisiana Workforce Commission (LWC) for employers and insurers to report all workers’ compensation expenses for a calendar year. The report captures costs paid by both the employer and insurer, including indemnity benefits (temporary, permanent, death), lump-sum settlements, medical and rehabilitation expenses, legal fees, and other related payments.
