Status Report On Conservatee {PRB-PCN-005}

PRB-PCN-005 - STATUS REPORT ON CONSERVATEE. This form is required by the Superior Court of California, County of San Francisco, for all conservators of the person—one year after their appointment and every two years thereafter. It is used to provide the court with a comprehensive update on the conservatee’s current condition and living situation. The form must be completed in full, and if any item does not apply, the conservator must indicate so with “not applicable” or “none.” It collects detailed information on the conservatee’s living arrangements, physical and mental health, medical treatments received, medications taken, social and recreational activities, and visitation schedule.
