DESIGNATION OF E-MAIL ADDRESS FOR A PARTY NOT REPRESENTED BY ATTORNEY. This form is used in the Florida District Court of Appeal by self-represented individuals (also known as pro se parties) to formally provide their email address for the purpose of receiving official court documents electronically. In accordance with Florida Rule of General Practice and Judicial Administration 2.516(b)(1)(C), this designation authorizes the court, clerk of court, and all other parties in the case to serve legal documents—such as motions, orders, pleadings, and other written communications—via email or the Florida Courts E-Filing Portal. The form requires the party to list both a primary and, optionally, a secondary email address, and acknowledges the responsibility to keep that information up to date with the clerk’s office.
