PBIP16f - STATEMENT OF INFORMAL APPOINTMENT OF PERSONAL REPRESENTATIVE (Person Died Without A Will – “Intestate Estate”) OR STATEMENT OF INFORMAL PROBATE OF A WILL AND INFORMAL APPOINTMENT OF A PERSONAL REPRESENTATIVE (Person Died With a Will – “Testate” Estate”). This form is used in the Superior Court of Arizona in Maricopa County to record the court’s decision to appoint a personal representative to administer a deceased person's estate. The form may be used when the person died without a will (intestate) or with a will (testate), and it documents the findings of the Probate Registrar after reviewing an application for informal probate or appointment. If the court finds that all legal requirements under Arizona law have been met—including proper application, entitlement to appointment, and validity of a will if one exists—it grants informal probate and/or appoints the requested individual as the estate’s personal representative.
