This form is used in the Superior Court of Arizona in Maricopa County to begin the probate process after someone has passed away. It allows an eligible person—such as a surviving spouse, adult child, parent, sibling, creditor, or someone named in a will—to request appointment as the personal representative of the deceased person's estate. The applicant must indicate whether the decedent died intestate (without a will) or testate (with a will), and provide information about the decedent’s death, residence, and surviving family members or heirs. The form includes statements about whether a will exists, the applicant’s legal right to serve as personal representative, and any other parties with equal or greater priority to be appointed.
