Property owners, or their authorized representatives, use this form to notify the Chief County Assessment Office of a change in mailing address to ensure they continue receiving important tax and property-related correspondence. If the property is held in the name of a trust, LLC, corporation, or other legal entity, supporting documentation identifying the authorized signatory must be attached. Owners must indicate if the property is no longer their principal residence, which will result in the removal of all exemptions as of the following January 1.
