MCR-103 - REQUEST TO CALENDAR CASE. This form from the Superior Court of California, County of Mendocino is used to formally ask the court to place a criminal case on the calendar for a new hearing date. This request can be submitted by the defendant, their attorney, a deputy district attorney, a probation officer, or a court clerk, and applies whether the defendant is in custody or out of custody. The form allows the requesting party to select the reason for the hearing, which may include a violation or modification of probation, a request to terminate probation, a request to modify sentencing terms such as extending time to pay a fine or complete community service, or changes related to DUI programs or ignition interlock requirements.
