AK LBF 37A - DECLARATION RE: ELECTRONIC FILING [INDIVIDUALS]. This form is used in the United States Bankruptcy Court for the District of Alaska by individual debtors who are filing for bankruptcy with the assistance of an attorney. The form serves as a sworn declaration by the debtor(s) confirming that all information provided to their attorney, as well as the contents of the electronically filed documents—such as the bankruptcy petition, schedules, statements, creditor matrix, and other required forms—is or will be true and correct. The debtor(s) also give permission for their attorney to electronically submit these documents to the court. This declaration must be filed within 14 days of the electronic filing of the petition; failure to do so may result in the case being dismissed after a short-notice hearing. If the case is filed under Chapter 7 and involves primarily consumer debts, the form includes a statement acknowledging that the debtor(s) understand the different chapters under the Bankruptcy Code and have chosen to proceed under Chapter 7.
