LASC PRO 046 - NOTIFICATION OF MINOR’S DEATH. This form is used in the Superior Court of California, County of Los Angeles, to formally notify the court and relevant parties of the death of a minor who was under a guardianship. The form must be completed by the guardian, the guardian’s attorney, or another interested party and includes the date of the minor’s death. If there was a guardianship of the estate, Probate Code Section 2467(a) states that the guardian must continue managing the minor’s estate until it is transferred to the personal representative of the minor’s estate or otherwise disposed of according to the law. The form also requires that copies of the notification be served to all individuals entitled to notice under Probate Code section 1460, including those who have requested special notice.
