FORM 18.4 - JUDGMENT ENTRY FINDING CONSENT NOT REQUIRED. This form from the Probate Court of Stark County, Ohio, is used in adoption proceedings when the court determines that a parent's or other individual's consent to the adoption is not legally required. Under Ohio Revised Code (R.C.) 3107.07, the court may find consent unnecessary if the parent has failed, without justifiable cause, to have more than minimal contact with the child or to provide financial support for at least one year before the adoption petition was filed or before the child was placed in the petitioner’s home. Additionally, other legal grounds under R.C. 3107.07, such as the status of a putative father of a child born before January 1, 1997, may apply.
