Description

FORM 18.2 - NOTICE OF HEARING ON PETITION FOR ADOPTION. This form from the Probate Court of Stark County, Ohio, is used to formally notify relevant parties of an upcoming court hearing regarding a minor's adoption. This notice must be served at least 20 days before the hearing and includes the date, time, and location of the proceeding. The form also states whether the consent of a biological parent is required under Ohio Revised Code (R.C.) 3107.07. If the court finds that a parent has failed, without justifiable cause, to maintain contact or provide financial support for the child for at least one year before the petition was filed, their consent may not be required. The notice informs the recipient that if the adoption is granted, all parental rights and responsibilities will be terminated, severing legal relationships between the child and their biological relatives (except for a spouse of the adopting petitioner and that spouse’s relatives). It warns that failure to file an objection within 14 days after receiving the notice or failure to appear at the hearing may result in the adoption being finalized without their input. The notice also advises the recipient of their right to an attorney and provides court contact information for those unable to afford legal representation. The form includes a certification section for court officials to confirm that the notice was properly mailed via certified mail, as well as a return of service section for law enforcement officers to document the delivery of the notice. www.FormsWorkflow.com
