FORM 15.5 - GUARDIAN'S INVENTORY. This form is used in the Probate Court of Stark County, Ohio to provide a detailed accounting of all real and personal property belonging to the ward at the beginning of the guardianship. Required under Ohio Revised Code § 2111.14(A), this form helps the court oversee and protect the ward’s assets. The guardian must list and assign values to all personal estate (e.g., bank accounts, vehicles, investments), real estate (e.g., homes, land), and the annual income or rental income the estate produces.
