FORM 15.2 - FIDUCIARY’S ACCEPTANCE – GUARDIAN. This form is used in the Probate Court of Stark County, Ohio when a person accepts the legal responsibilities of serving as a guardian for another individual, referred to as the ward. The form outlines the guardian’s required duties under Ohio Revised Code § 2111.14 and must be signed by the appointed guardian to formally acknowledge and agree to these obligations. If the guardian is appointed over the estate of the ward, they agree to tasks such as filing an inventory of the ward’s assets within three months, depositing and investing funds lawfully, filing periodic financial accounts, preserving any wills, and spending funds only with court approval.
