Description
DC 6:11.5 - DECREE FOR NAME CHANGE OF A MINOR. This form is used in Nebraska District Court to authorize the legal change of a minor child’s name. This form is issued after a hearing on a petition filed by a parent or guardian requesting the name change. It confirms that the court has jurisdiction over the matter, that the petitioner and the child have been bona fide residents of the county for the required period, and that any publication or notice requirements under Nebraska law have been met. The decree identifies the child’s current name and the new name requested, establishes that there is proper and reasonable cause for the change, and orders the minor’s name to be changed to the new name as approved by the court. Neb. Rev. Stat. § 25-21, 271. www.FormsWorkflow.com
