This form used by the State of Vermont Superior Court, Probate Division is a legal document that a newly appointed guardian must complete to formally accept their fiduciary responsibilities and to secure their performance of those duties. This form establishes a financial obligation—referred to as a bond—that may be set either with or without surety, depending on court requirements. The bond acts as a guarantee that the guardian will faithfully carry out their responsibilities, which include filing a complete inventory of the respondent’s property within 30 days, managing the estate in the respondent's best interest, submitting annual financial accounts and personal status reports, and properly distributing remaining assets upon termination of the guardianship. If the guardian fails to meet any of these obligations, the bond ensures that funds are available to cover any resulting losses or damages.
