This form is filed with the Indiana Secretary of State in accordance with Indiana Code 23-0.5-4-9 and does not require a filing fee. The registered agent may use this form either to voluntarily resign their appointment or to declare that they never consented to being appointed in the first place. The form requires the name and address of the business entity, the name of the registered agent, and the appropriate resignation statement to be selected.
