MEDIATION ORDER. This form is used by the Circuit Court of the Second Judicial Circuit in Illinois to refer a case to mediation. The court orders mediation for specific issues such as allocation of parental responsibilities, parenting time, or the removal of a child from the jurisdiction. The form appoints a mediator, either an approved mediator of the circuit or a judicial mediator, and specifies the payment arrangement for the mediation. It also sets a date for a continued case management conference, at which the mediator is to report the status of the mediation.
