JC 53 - NOTICE TO PUTATIVE FATHER. This form is used in Michigan's Family Division. It informs an individual that they may be the natural father of a child involved in a court case. The form indicates that a petition seeking court jurisdiction over the child has been filed. The putative father is required to appear at a specific date and location to state their interest in the child. Failure to appear may result in the denial of any interest in the child and waiver of notice for subsequent hearings. The form also mentions the possibility of termination of any rights to the child. It highlights that accommodations for disabilities or foreign language interpretation can be arranged by contacting the court. If the court finds probable cause that the individual is the child's father, notice will be served through appropriate means, including publication if the father's whereabouts are unknown. Approved, SCAO. MCR 3.921(D).
