PRO 057 - APPLICATION FOR APPOINTMENT TO COURT APPOINTED COUNSEL PANEL CONFIDENTIAL – FOR COURT USE ONLY. This is a Los Angeles County Superior Court form used by attorneys seeking appointment to the Probate Court Appointed Counsel Panel. Through this application, attorneys certify compliance with California Rules of Court rules 7.1101–7.1105 and Los Angeles Superior Court Rules 4.123–4.127. Applicants must submit Judicial Council Form GC-010 (Certification of Attorney Qualifications), proof of professional liability insurance, and proof of required education.
