This is a San Francisco Superior Court family law form used when a party has received assistance preparing a proposed judgment package from a consulting attorney, mediator, legal document preparer, or legal document assistant. The form authorizes the court and court personnel to communicate directly with that individual regarding filing issues or questions related to the judgment package. It also records the preparer’s contact information and allows the filing party to later withdraw consent by filing a separate notice.
