ACR 406 - APPLICATION FOR CERTIFIED COPY OR SEARCH OF A DEATH RECORD. This is a Riverside County Assessor-County Clerk-Recorder form used to request an authorized or informational certified copy of a death certificate, or a search resulting in a Certificate of No Record. The current fee is $24.00 per record. Applicants must provide the decedent’s name, date of death, city of death, and number of copies requested. Authorized certified copies are restricted to eligible individuals under California Health and Safety Code §103526, such as immediate family members, legal representatives, or certain government officials, and require a sworn statement.
