This is a Los Angeles Superior Court family law form used to identify and disclose additional contracts related to a minor’s earnings or services when filing a Petition for Approval of Minor’s Contract. Required under Family Code §§ 6750–6751 and Labor Code § 1700.37, this attachment ensures the court reviews all related agreements affecting the minor.
