MED-150 - REQUEST FOR COURT-SPONSORED MINOR GUARDIANSHIP MEDIATION. This form is used in Alaska's Superior Court to ask the court to order mediation in a minor guardianship case. This form is typically submitted by a petitioner, parent, attorney, Guardian ad Litem (GAL), guardian, or another party involved in the case. The purpose of the mediation is to help the parties involved—such as parents and guardians—discuss and potentially resolve disputes regarding the care and guardianship of a minor without the need for a full court hearing. The form requires the requesting party to identify themselves and their relationship to the minor(s), provide potential dates and times for mediation, and list the people who should participate in the process. It also includes a section addressing domestic violence concerns, as Alaska law limits the court’s ability to order mediation if domestic violence has occurred between the parties. If there is a protective order in effect, or if domestic violence has occurred but the victim has agreed to mediate, this must be disclosed.
