LASC FAM 174 - ADDITIONAL MINORS—ATTACHMENT B TO PETITION. This is a Los Angeles Superior Court family law form used in connection with a Petition for Approval of Minor’s Contract under Family Code §§ 6750–6751 and Labor Code § 1700.37. This attachment is required when more than one minor is involved in the petition. For each additional minor, the form requires the minor’s name, birthdate (with a copy of a state-issued birth certificate, passport, or driver’s license attached), state of residence, and identification of the guardian ad litem (including relationship and supporting court documentation if applicable). The form also requires disclosure of any lender or loan-out company for the minor’s personal services, with reference to Attachment F (FAM 178) if applicable. For Mandatory Use.
