Form 36 - WAGE DEDUCTION SUMMONS. This form is used in the Circuit Court for the Seventh Judicial Circuit of Illinois, Sangamon County. It is issued to an employer when a judgment has been entered against an employee (the defendant) to begin the process of garnishing wages.
